This role involves driving membership growth for the Accountable Care Organization (ACO) solution by orchestrating and executing growth strategies in collaboration with growth, ACO, market enablement, and market leadership teams. The focus is on identifying priority markets, building and managing the ACO growth pipeline, and expanding strategic partnerships with SNF operators to increase ACO enrollment in senior living communities. The position is fully remote with up to 50% travel.
Responsibilities
- Identify and pursue new opportunities to increase ACO participation through prospecting, outreach, and solution presentations.
- Partner with growth, ACO, and markets teams to develop a targeted growth pipeline to achieve membership goals.
- Coordinate with teams to integrate the ACO solution into new and existing relationships.
- Develop and execute growth strategies leveraging provider networks and SNF partnerships.
- Achieve enrollment targets through proactive outreach and relationship management.
- Assess SNF partner needs and present tailored solutions aligned with clinical and financial goals.
- Facilitate partnership discussions and agreements supporting shared success in Medicare Advantage (ISNP), ACO, and other value-based arrangements.
- Build consultative relationships with medical groups, long-term care facilities, and senior living communities.
- Present program benefits to decision-makers to drive adoption.
- Conduct competitive analysis and market assessments to identify growth opportunities.
- Refine ACO and integrated ISNP value proposition based on market feedback.
- Track and report on growth metrics, pipeline progress, and conversion rates.
- Collaborate on onboarding and implementation for new partners.
- Maintain compliance with regulatory requirements and accurate records using CRM tools.
- Provide insights on performance and market trends to leadership.
Qualifications
- Valid state driver’s license with a good driving record.
- Proof of automobile insurance per minimum required by home state.
- Auto insurance coverage as required by state of residence.
